Physical Therapist
(1 Full-time, Benefited & 1 Part-time Position)

Job Duties:

• Assess client need and provide physical therapy services intended to improve mobility and strength, decrease pain, and prevent injury according to written physician orders
• Function as part of the Home Care team relative to the provision of physical therapy services
• Maintain documentation ensuring up-to-date client record, compliance with all state and federal requirements and in accordance with accepted standards of practice
• Collaborate effectively with other members of the care team

Qualifications:

• Master’s degree in physical therapy from an accredited physical therapy program
• Licensed to work in Minnesota as a Physical Therapist
• Strong knowledge of physical therapy principles, theory, methods, and evidence based practice
• Minimum of 2 years experience
• Strong interpersonal skills
• Must be able to provide insured vehicle for transportation with a valid driver’s license
• Good communication and problem solving abilities
• Organized, self motivated, and self directed- able to work independently
