Position Purpose:

Opportunity to work with pregnant women/teens and new mothers in their home/schools. Nurse will provide client with education and support related to prenatal care, newborn care & parenting skills. Clients are culturally diverse and reside in Minneapolis and suburban Hennepin County.

BSN/PHN Assessment Nurses
Multiple Part-Time, Non-Benefited Positions

Position Purpose:

BSN/PHN Nurses conduct assessments to determine PCA services needed for clients living in the eleven-county Twin Cities area.

Qualifications:

  • BS in Nursing required. Successful candidates must hold or be elegible to apply for PHN certification in Minnesota. Eligible for RN licensure in the state of Minnesota.
  • Skilled nursing with PCA assessment experience preferred.
  • Legible handwriting
  • Certified in CPR
  • Good communication and problem solving abilities
  • Must be able to provide insured vehicle for transportation with a valid driver's license

 

Child Care Nurse Consultant
Part-Time Position

Position Purpose:

We have an immediate opening for a part-time Child Care Nurse Consultant. As a Consultant PHN, working flexible hours, you will make on-site visits, assess and make recommentations for greater Metro area child care centers in accordance with health program needs and compliance with Rule 3. This position offers you the opportunity to make a difference in the lives of children and staff.

Qualifications:

  • Current PHN certificate and RN license in Minnesota
  • Minimum of two years of nursing experience
  • Prefer strong background in early childhood and childcare consultation
  • Flexibility with good time management skills
  • Ability and desire to provide child care consultations and guidance to staff within diverse & multi-faceted environments

RN-BSN Case Coordinator
Multiple full-time, benefited and part-time non-benefited positions

POSITION PURPOSE: 

The practice of the RN Case Coordinator will focus on helping clients access and negotiate complex delivery systems, arrange and schedule services, facilitate communication among multiple providers and monitor health and wellness changes.  Clients are culturally diverse and reside in the seven-county metro area. 

QUALIFICATIONS:

  • Must be eligible for licensure as a Registered Professional Nurse in Minnesota
  • Minimum 2 year RN degree, BSN strongly preferred
  • Minimum 2 years of nursing experience in ambulatory care setting, such as clinic, home care agency, or hospital working as part of a multidisciplinary team.
  • Strong understanding of Case Coordination and the ability to implement a structured client care process allowing for assessment, development of a plan of care and evaluation.
  • Experience with a documentation system specifically designed to optimize therapeutic outcomes.
  • Certified in CPR
  • Must be able to provide insured vehicle for transportation with a valid driver’s license
  • Proven ability to prioritize multiple tasks and projects, and under specific deadlines
  • Good communication and problem solving abilities
  • Organized, self motivated and self directed, able to work independently.

 

CLINICAL DIRECTOR – HOME HEALTH
1.0 FTE

 

POSITION PURPOSE: 

The position will be responsible for the over-all operations of the Home Health Division and lead a strong nursing team with the purpose of providing high quality nursing care services.  Will design and update programs and systems that ensure productive cost efficient home health visiting services for the community. The leader will participate in planning, implementation and monitoring of client care services.  Activities include supervision of staff, day-to-day program management, tracking of outcomes as well as the implementation of agency initiatives including a strong emphasis in quality of services delivered and a high level of staff professionalism.
 
           
QUALIFICATIONS:

  • BSN and PHN certification required, Master degree or higher preferred
  • Licensed or eligible for licensure as a Registered Professional Nurse (RN) in Minnesota
  • Certified in CPR
  • 5years demonstrated leadership & supervisory experience in home health or public health nursing, hospital or long term care setting
  • Operational experience in budget management, program development, teaching and supervision and regulatory compliance
  • Exceptional critical thinking and problem solving skills with ability to work within team-based, high energy and fast paced environment
  • Organized, self-motivated and self-directed
  • Strong verbal and written communication skills
  • Demonstrate sensitivity to working with people of different races and cultures
  • Must be able to provide insured vehicle for transportation with a valid driver’s license

 

NURSE (LPN) CUSTOMER SERVICE/INTAKE & PRIOR AUTHORIZATION REP
Full-Time – 1.0 FTE

 

POSITION PURPOSE: 

LPN to perform Customer Service for Intake and Prior Authorization for MVNA.  Communicate effectively with internal and external staff, health care providers and insurance companies.  Coordinate with various departments within the agency to support intake services including eligibility and payment sources.  Problem-solve and data enter a variety of information into the computer system.  Be able to navigate through many systems and understand and articulate the information on the screens.  Position requires working 1-2 week-ends per month and is an in-house position.

QUALIFICATIONS:

  • LPN with current license in Minnesota
  • Excellent communication and customer service skills
  • Ability to communicate effectively by telephone.
  • Detail oriented and able to prioritize a heavy workload
  • Good problem solving and decision making abilities
  • Minimum of one year experience in word processing and data entry procedures with high level of accuracy.  Experience working in databases preferred.
  • Intake experience or health care authorization experience preferred – ideally in homecare

 

Nurse Family Partnership PHN
1 Full-time benefited position available

POSITION PURPOSE: 

To provide long term home visiting services to first time mothers in Minneapolis using the Nurse Family Partnership (NFP) model.  NFP is an intensive home visiting model that has been rigorously researched with documented outcomes using a well defined approach. The nurse home visitor will be expected to provide services that align with a prescribed visiting schedule, use approved tools and document assessments at designated time frames.  The nurse home visitor and the supervisor will engage in case conferences on a regular basis, using reflective practice principles.  Clients will be identified as those at highest risk in Minneapolis and will include teens as well as other first time mothers.    

QUALIFICATIONS:

  • Must be eligible for licensure as a Registered Professional Nurse in Minnesota
  • Certified PHN or eligible for PHN certification in Minnesota
  • One or two years of related nursing experience preferred
  • Certified in CPR or MVNA to recertify
  • Must be able to provide insured vehicle for transportation with a valid driver’s license
  • Good communication and problem solving abilities
  • Organized, self motivated and self directed, able to work independently
  • Willingness to follow NFP guidelines and maintain fidelity to the model in all aspects of the work
  • Ability to act as a representative of MVNA out in the community and build partnerships with community organizations
  • Able to attend 1 week of training out of state
