Block Nurse Programs

MVNA collaborates with block nurse programs to provide specialized geriatric services in Hennepin and Ramsey County neighborhoods. We provide holistic home health care, a nurturing enviroment that considers the management of physical, pshchological, social and spiritual concerns of elderly clients.

Block Nurses are Registerd Nurses (RN) with specialized training in public health and geriatrics. The team also includes Licensed Practical Nurses (LPNs) and Home Health Aides. Services include skilled nursing health assessment and instruction, nutrition assessment and instruction, exercise instruction, home safety evaluation, client advocacy, medication management and personal care including hygiene and mobility.

How to activate services:

A doctor's order is required to receive services. Your doctor can call an order in by telephone, to our Intake Department at 612-617-4700, or the doctor's order can be faxed to 612-617-4781.
